We are seeking a Clinical Care Coordinator to join an Early Hospital Discharge Team.

OPPORTUNITY
To join a new exciting Service, overseeing early discharge from hospital to home and home nursing care.
The successful candidate would coordinate this innovative regime providing nursing care for patients in their own homes who would otherwise be in hospital. This Service caters for patients who have undergone elective surgery, unplanned surgery as well as hospital admissions for medical conditions.
You will provide effective clinical and managerial leadership to nursing teams and support staff ensuring the effective delivery at all times of high quality nursing care.
WHO WOULD THIS ROLE BE GREAT FOR?

NMC registration

Minimum 2 years experience managing teams (essential)

Experienced with Cannulation, Venepuncture and IV Therapy

A background in Orthopaedics, Surgical or Community Nursing would be advantageous

Car driver (essential)
SALARY
Salary Range £35,000 - £40,000 (depending on experience)
